We provide timely and informative feedback to referral sources and believe communication between referral source, clinician, and patient is crucial to successful rehabilitation and appropriate case management.
Referral sources receive:
- Quick and convenient scheduling
- Initial evaluation and plan of care report
- Progress notes every 30 days: including progression towards functional and objective goals.
